Please complete this form to order the 65th Annual IASN Conference T-shirt. Cost is $15 plus a $10 fee for shipping. If you are going to the School Nurse Lobby Day, your T-shirt will be given to Bridget Heroff and distributed on the day of the event. The shipping charge will be waived. If you are not going to the lobby day, the T-shirt will be shipped to your home. Sizes are limited. You will be notified if your size is not available.
